  Renaming theme keeps old/new theme in Theme manager

Bug description:
  Windows 7, OpenLP 1.9.12 build bzr2084

  When renaming a theme, after the rename completes, both the old and
  new name theme are in the Theme Manager.

  If you attempt to delete the old theme, a traceback occurs.

  Traceback (most recent call last):
    File "D:\OpenLP_Code\trunk\build\pyi.win32\OpenLP\out00-PYZ.pyz\openlp.core.ui.thememanager", line 349, in onDeleteTheme
    File "D:\OpenLP_Code\trunk\build\pyi.win32\OpenLP\out00-PYZ.pyz\openlp.core.ui.thememanager", line 362, in deleteTheme
  ValueError: list.remove(x): x not in list

  The old theme can be selected as "default", but it used, the "Default"
  black theme is displayed.

  Attached is a screen shot.

  The theme "Baptize" was renamed to "Baptize them"
