In the Print Service Options there is a text box that is labeled "Custom
Footer Text".  When text is added to the text box it shows up with a
header of "Custom Service Notes:" and it only shows up at the end of the
service print document.

The option screen should be change to say "Custom Service Notes:"

to recreate:
1. select File from the menu
2. select Print Service from the drop down menu
3. select Options in the Print Service window
4. Add text to the Custom footer Text box
5. select Options in the Print Service window and review the information in the window.  The Custom Service Notes will be at the end of the window and will contain the text you entered.
