If you need a very fast solution, a Google form can be quickly embedded
on any page anywhere, and making such a form is very non-technical. Once
created it can immediately be added to a Wordpress page and given to the
world.

For the long term, this is a typical application for CRM software, which
we have at beta.theosdf.org.

  I've gone and obligated us[1], though this is something a few of us
  have talked about for a while.  Kevin[2], our resident policy geneus,
  suggested it contain the following:

  Name (of owner)
  Business
  Business Address
  Business Phone number (for verification)
  Industry
  Can we use your business name in OSDF publications in the future?
  ------------------------------------------------------------------------

  I'd be happy to build a mock up if this is something we can up and
  running in the next 5 days or so.
