Bug description:
  When you try and change a menu item to a new template/theme from the Manage Menus screen, you get the following:
  Undefined index: new_window_array (/home/mydomain/phpds/plugins/PHPDevShell/controllers/menu-admin/menu-item-admin-list.php line 60 )
