[Bug 506407] Re: order form is invalid html
this code was cut and pasted from the previous wiki order form so it
should work fine as before unless my cut and paste did something odd

Bug description:
In download.html the html for the order form is a bit messed up - I'm scared to touch it in case I break it (btw has someone checked that it works?)
