Sorry, not too much on this problem. It reviewed the code, tested it
but could not reproduce the crashes. Which means I could not debug
them. So honestly, I ain't got a clue.

One thing I could think of is that prefetching interrupts rebuilding
the document if this takes more than 500 ms, so I made sure it is
suppressed. This just landed in trunk, but it's more or less
guesswork. Testing is appreciated.
