This regression happened between commits a8cbb156 and dc9de806 mesa master. That is from one xorg-edgers daily checkout (20090901) to the next (20090902) and the best I can do to pinpoint it.

I have discovered another visual phenomenon that seems to coincide with
the non-showing buttons. In blenders outliner window some icons are
offset by one. See

correct behaviour and

wrong behaviour. I can however not pinpoint when this other phenomenon
showed up with the same precision. They might not be the same bug.

monitor, not on the internal. I can confirm that that bug has been fixed
recently.

This bug here is a different bug. 
I usually run the latest ubuntu karmic with xorg-edgers ppa, which checks out mesa master daily.
Just now I checked with:
mesa (7.7.0~git20090914.d3f5a2e5-0ubuntu0tormod) karmic; urgency=low
  * Checkout from git 20090914 (master branch) up to commit
    d3f5a2e5ab413ec0dab1efa8135705e5312d361d
the bug is still there.
