RaiMan proposed the following answer:
If Region(613,580,166,23) is the region, that contains the wanted
checkbox, then this would do it

Region(613,580,166,23).click("1375270632896.png")

or the same:
r = Region(613,580,166,23)
r.click("1375270632896.png")

or similar:
click(Region(613,580,166,23).find("1375270632896.png"))

which takes care, that the search in the click only looks in
Region(613,580,166,23)

Your approach:

Region(613,580,166,23) # this does nothing of value
click("1375270632896.png") # this still searches the screen
