Loran gave more information on the question:
Sikuli version 4.0.1, on Windows 7. One thing I noticed was when it
works correctly, the console says something (going off memory, can get
exactly if needed) like:

        [log] App NAME OF APPLICATION 0 #0 focus

But when it fails the number after the application is a large one:

        [log] App NAME OF APPLICATION 2137 #0 focus

Could there be hidden windows or something in the background?
