you have to file pulseaudio since it cannot find your second headphone jack

pulseaudio expect [Element Headphone2] headphone2 playback
volume/headphone2 playback switch

but driver create headphone playback index 1

this mean you also need to file kernel bug report about the name of the
playback switch of second headphone is not pulseaudio expect

Title:
  Low volume with headphones

Status in pulseaudio package in Ubuntu:
  Incomplete

Bug description:
  I've made a clean install of Ubuntu 15.04 but I'm having this issue
  when using headphones. Volume is very low, even when turning it to
  maximum. Everything worked fine under Ubuntu 14.04 that I was using
  before 15.04.

  I've reset the audio configuration with the command "rm -r
  ~/.config/pulse; pulseaudio -k" but it only fixes the issue
  temporarily. After each restart, the issue happens again and I have to
  execute the above command to get normal volume with headphones.

  Volume is normal with built-in speakers however.
