since there are only two playback volume control ,

either

two headphone share volume control and speaker has it own volume cintrol

or


speaker and headphone share volume cintrol and headphone spdif  combo has it own volume control


the name of the second headphone playback switch should allow user know
which headphone jack

  Low volume with headphones

Status in pulseaudio package in Ubuntu:
  Incomplete

Bug description:
  I've made a clean install of Ubuntu 15.04 but I'm having this issue
  when using headphones. Volume is very low, even when turning it to
  maximum. Everything worked fine under Ubuntu 14.04 that I was using
  before 15.04.

  I've reset the audio configuration with the command "rm -r
  ~/.config/pulse; pulseaudio -k" but it only fixes the issue
  temporarily. After each restart, the issue happens again and I have to
  execute the above command to get normal volume with headphones.

  Volume is normal with built-in speakers however.
