Bug description:
  Reported by an OEM who's phone currently does have such an icon, there
  is no visual indication that a wired headset has been connected to the
  phone and in use:

  [Actual results]
  There is no earphone icon when inserting the earphone

  showing the icon, and volume, of the primary output for the current
  active role. (For example, when you plug in headphones, a notification
