Syntax of index ranges in the manual and their translation

Hi all,

I have done an update [1] of the syntax of the index ranges. Until now
we had to write

\index{Ubuntu!installing|(}
[...]
some text
[...]
\index{Ubuntu!installing|)}

to get an index range.

Starting now we have to write

\indexstart{Ubuntu!installing}
[...]
some text
[...]
\indexend{Ubuntu!installing}

to get the same result.

The advantages are that we now have only one string in the translation
file which is used by \indexstart and \indexend in the same way. This
avoids typos. Also, we do not force the translators anymore to
translate two almost identical strings twice.

This affects 47 strings in our manual, so we have 47 strings less in
the translation :-)
