Yes, there are a number of indicator bugs that have been filed about SIM PIN unlocking, and the white-screen issue is being worked on:
https://bugs.launchpad.net/ubuntu/+source/indicator-network/+bug/1336675We've seen problems with 3G not being re-established properly coming out of FlightMode, however this seems different than what you're describing.
I would suggest entering a network-manager bug for this last problem. Please include the image # ( use system-image-cli -i for this ) in your bug, and the exact steps to reproduce if you can narrow down one or more scenarios that trigger the problem.
