I have had this already twice too with the E4.5 r22 of my wife (mine does
not show this, but I have another use profile, mine is all day attached
to my laptop from 7 to 22); the only new in your case is, that it
happens even while attached to power; but maybe after "full" the
charging is just switched off and it behaves like not attached to a
charger. I filed a bug issue (I do not have the number, search for it,
please). You should attache your case there.

BQ.com's support said to me that I should re-calibrate the battery. Which I now
have done with 4 times completely drained and charged to 100%. We will
see if this help.

Maybe it helps running a script by crond and raise alarm, if battery is
below 10%.
