  improve the simple html5 app template

Bug description:
  The current HTML5 template proposes a sample address book using the
  Ubuntu HTML5 widgets. The idea is to improve that template by
  simplifying it, by including pointers to available documentation to
  better welcome the developer, and to give him or her the tools to get
  started with the Ubuntu HTML5 platform.

  The template will be modified to include a few buttons that will point
  to the available documentation for the html5 platform and to the
  current apis. A dedicated CSS file will also be created (instead of
  inlining the style in the HTML file). The template needs to be easy to
  break down so the developers can replace its content with their own
  code in no time.
