Another approach to fixing (2) is to uncheck this in the save to /etc/X11/xorg.conf dialog:
    [ ] Merge with exiting file.

It's become apparent that enabling Xinerama makes the main VGA desktop wider.
Whether or not I enable Xinerama - still no action on the second monitor (No Signal).

I've attached the xorg.conf generated by nvidia-settings.  I'm left
wondering if there's some way it's confusing X into thinking there's
only a single monitor.    There are two Monitor sections (0 and 1) and
exactly two Screen sections corresponding to them. The only mention of
Screen0 and Screen1 is in the main "ServerLayout" section:

Section "ServerLayout"
    Identifier     "Layout0"
    Screen      0  "Screen0" 0 0
    Screen      1  "Screen1" 1024 0
    InputDevice    "Keyboard0" "CoreKeyboard"
    InputDevice    "Mouse0" "CorePointer"
EndSection

The mouse and keyboard effect screen 0, but not screen 1.
Screen1 is forever plagued with "No Signal".
X has allocated the width for Screen1 in Screen0's desktop.

Can this be a hardware problem?
How can help isolate the failure?
