I am fairly confident that the actual cause of the bug is emacs and this
ticket is probably a duplicate of:

Which is explains why I mostly saw it at work, when I have emacs open!

Turning off assistive technologies seems good so far.  Will test over
the rest of the week and file this as a duplicate if the problem is
gone.
