Exactly same problem with Karmic 9.10 x86_64, xserver-xorg
1:7.4+3ubuntu10.

All parts of Stilians description match: "When using the middle mouse
button to paste or open new tabs, nothing happens. Pressing left+right
results in the expected action. xev confirms that no event is generated
for the middle button."
