Compiz has a plugin that can prevent windows from getting offscreen. It does not restrict the cursor though, and I don't know about desktop icon placement.
Activate the "Put window" plugin, go to "Misc. Options" and enable "Avoid Offscreen".

It would be nice if this functionality could be implemented for the cursor (and icons) too.
Other than that, Bug #58977 also needs to be fixed to get dual-head working properly.
