Sebastian Gurkasch proposed the following answer:
It will only remove packages, that are no longer useful for your system,
e.g. when they were installed as dependency for a program you installed.
It is not going to remove programs (or their dependencies) you explicit
installed.
Of course, it is just a program and isn't unfailing, so take a good look
at the list before cleaning up.
