[HG UFL] Added (optional) arguments name and coefficient_names to FormData.

Added (optional) arguments name and coefficient_names to FormData.
Default values "a" / "w%d" % i are chosen if not provided.

Made load_forms return a list of FormData instead of (name,form) pairs.
