[HG UFL] Cleanup of extract_indices, split it into one

Cleanup of extract_indices, split it into one
version used by product and one by indexed,
instead of the conditional behaviour which
was harder to follow.

There's still a bug in the AD, but it is related to something else.

Some fixes in spatial derivative index handling
(fixed ugly bug, only supporting single derivative
in one SpatialDerivative object now)
Added some more tests. More tests pass, but
there are still problems.
