Google Wave and Picasa have an interesting solution to dynamic loading and
long items: Their scrollbars control velocity rather than position, for
example if you drag very far down the scrolling is very fast and when you
drag close to the initial position it scrolls slowly. A translucent bar
indicates visually where you are with respect to the whole.

There are also arrows on the widget to page up and down. It's hard to
explain, but easy to comprehend once you try it out.
