  Same error message in LBaaS form is displayed twice

Status in OpenStack Dashboard (Horizon):
  Fix Released

Bug description:
  When some error occurs in Load Balancer form, the same error message is displayed twice.
  For example, if "Add Vip" fails, we see two error message but both have a mesasge with 'Unable to add Vip "vip1".'
