Title:
  No unique in `console_pools` table

Status in OpenStack Compute (Nova):
  Fix Released

Bug description:
  In `console_pool_get_by_host_type()` function we assume that there is
  no duplicate values are entered in columns  (`host`, `console_type`,
  `compute_host`) but we can add duplicate values to these columns in
  `console_pools` table.
