Boot from volume fails in Horizon

Bug description:
Trying to launch a new instance through Horizon, get the following
errors:
"The server has either erred or is incapable of performing the requested operation. (HTTP 500)"
and
"Unable to launch instance named <name>"
and the new instance shows up in the instance list, stuck with "Build" status and "Scheduling" task.
There are no errors in any of the log files on the cloud controller or the nova/cinder node - and booting from the command line works as expected (as does booting from an image both in Horizon and via CLI).
