[Bug 1207874] Re: Unclear how to register and enable extensions
"hard for an admin to know" is a problem that can be solved by great
documentation, not by introducing redundant configuration options. in
the case of API extensions, document the dependencies a particular piece
of middleware has, such that it's a trivial task to add it to an
existing pipeline.

Bug description:
Extensions need to be enable in the paste.ini file but it is hard for
an admin to know where they should be injected.
Extensions should probably be enabled in the config file with an
on|off switch only and the extension code should tell where it wants
to inject itself in paste.
