Title:
  An instance created from an ami (converted to volume with
  block_device_mapping_v2) won't boot

Status in OpenStack Compute (Nova):
  Fix Released

Bug description:
  Booting an instance using the new block device mapping v2 API and
  using an AMI as source image with destination volume won't set the
  kernel and ramdisk values to the instance metadata. This will prevent
  the instance from booting.

  The following will create an instance that will get stuck at boot:

  $ nova boot --block-device
  id=<image_id>,source=image,dest=volume,size=1,bootindex=0 --flavor
  m1.nano instance

  where image_id is the id of an AMI image.
