Looks like there exists a duplicate of the bug

  Image members are not constrained by image_id & member_id

Status in OpenStack Image Registry and Delivery Service (Glance):
  Fix Released

Bug description:
  More than one image member for the same image with the same id can be
  created. This causes some unexpected behavior because a delete on the
  image member only deletes one of the duplicates. This means that a
  user could think they removed an image member but instead it is still
  there, a potential security issue because a user may accidentally
  allow access to an image.

  A fix for this will need to include a migration to handle duplicate
  image members. This could be tricky since any updates to a member may
  cause the duplicates to diverge. For example, updating status of a
  member when there are duplicates only sets one members status to
  'accepted'.
