  instance fault not created when boot process fails

Status in OpenStack Compute (Nova):
  Fix Released

Bug description:
  If the build process makes it to build_and_run_instance in the compute
  manager no instance faults are recorded for failures after that point.
  The instance will be set to an ERROR state appropriately, but no
  information is stored to return to the user.
