Public bug reported:

Instance boot from volume fails in Hyper-V if flavor has ephemeral disk.
Boot fails with the following error:

"HyperVException: WMI job failed with status 10. Error details: Failed
to add device 'Physical Disk Drive'."

This happens because the Hyper-V tries to attach both the ephemeral disk
and the boot volume to the same slot on the IDE controller.
