Discovered as admin user (as I realized lately, as demo user it behaves
the same as well), deployed by devstack with default conf. Navigate to
Admin - System - Images and fill form with these values:

Name: (leave empty)
Description: test
Image Source: image location
Image location: http://download.cirros-cloud.net/0.3.4/cirros-0.3.4-x86_64-disk.img
Kernel: cirros-kernel (4.7MB)
Ramdisk: cirros-ramdisk (3.6MB)
Format: QCOW2
Architecture: arch
Minimum Disk: 1
Minimum Ram: 1
Copy Data: no
Public: yes
Protected: yes

After hit Create Image, the form is not valid, "name" is missing, so the form is displayed again, but these fields of form lost old values:
Image location
Format

Used environment: devstack (latest commit has Change-Id:
