  Database Launch Instance should show Flavor Details

Status in OpenStack Dashboard (Horizon):

Bug description:
  The modal has a "Flavor" dropdown, but user has no idea what the
  Flavor is defined as.  We should follow the same convention has what
  the "Launch Instance" modal has.
