  Create Host Aggregate need a cancel button

Status in OpenStack Dashboard (Horizon):
  Invalid

Bug description:
  In Create Host Aggregate form, no cancel button. If user want to
  cancel the operation, they have to click the panel to go back to the
  dashboard.
