  api-ref: make example URLs consistent

Bug description:
  This bug covers the api-ref, namely, the stuff in api-ref/source in
  the glance repo.

  For consistency, make sure that example URLs for openstack components
  are in the form:

  project.openstack.example.org

  So, for example, an example image-list call to Glance would use a URL
  written like this:

  http://glance.openstack.example.org/v2/images
