Title:
  create port doesn't allow ip to be specified

Status in OpenStack Dashboard (Horizon):
  Invalid

Bug description:
  On Admin->System->Networks->[network detail]->Create Port there is no
  option to provide the ip address for the port to be created on.

  This exists in the corresponding API (neutron port-create) and should
  be avail in Horizon as well.

  Also, I'd expect this to be available in any Project level create port
  function that might be created (under discussion in bug
  https://bugs.launchpad.net/horizon/+bug/1399252)
