The current documents need to be updated to include create and assigning database rights.
This would apply to all the OS version pages.

create database glance;
GRANT ALL PRIVILEGES ON glance.* TO 'glance'@'%'  IDENTIFIED BY 'GLANCE_DBPASS';
GRANT ALL PRIVILEGES ON glance.* TO 'glance'@'localhost'  IDENTIFIED BY 'GLANCE_DBPASS';
flush privileges;
