  No instructions for creating database for glance

Bug description:

  This bug tracker is for errors with the documentation, use the
  following as a template and remove or add fields as you see fit.

  - [x] This doc is inaccurate in this way: The first step instructs the user to connect to the mysql database but does not provide the SQL commands to issue.
