    Add asterisk to conditionally required fields

Title:
  Subnet Network Address is missing asterisk

Status in OpenStack Dashboard (Horizon):
  Fix Released

Bug description:
  The Network Address field in the Create Subnet workflow (Subnet tab)
  is missing an asterisk.

  Steps to reproduce:

  1. Go to Admin -> Networks
  2. Click on a Network
  3. Click Create Subnet
  4. Attempt to create the subnet without filling in the Network Address field.

  The error message is at the top of the form, when it should be by the
  field, and the Network Address field itself is missing an asterisk.
