Title:
  Cannot customize header in login page

Status in OpenStack Dashboard (Horizon):
  Expired

Bug description:
  In login page, footer can be customized by using
  _login_form_footer.html[1]. However, header cannot be customized.

  It also should be customized as same as footer like
  _login_form_header.html.
