[Bug 2062137] [NEW] Install and configure controller node for Ubuntu in nova, openstack Block Device Mapping is Invalid

Public bug reported:

Using this documentation I created Openstack. Unfortunately I gets an
error when creating a new instance using Horizon.

- [ ] This is a doc addition request.
- [X] I have a fix to the document that I can paste below including example: input and output.

If I am wrong I apologize but it seems to me that the address mentioned should be different as:
"http://controller:5000/"; or "http://controller:5000/v3/";

In every place in this Guide is http and port 5000 for keystone hence I
do not understand why it is different here.
