A punctuation mark "?" at the end of a heading seems to break the
linkage between the TOC and the Note.

I can see the headings in the TOC, but the link does not work, and
the heading is not highlighted in the note.

E.g. 

==== When and how do you construct a random practice? ==== [does not
work]

==== When and how do you construct a random practice ==== [works]


As soon as I remove the ending "?" punctuation, the link works as
expected.
