Title:
  pressing submit freezes screen

Status in “gnome-panel” package in Ubuntu:
  New

Bug description:
  in the website page, i fill out information such as name, address,
  upload a photo, press submit and a message with a red x comes on
  screen saying "error, phone is required",  which makes no sense.  This
  freezes the screen.  I can not close the screen, close the file, it
  completely locks the screen.
