Public bug reported:

1) Install epiphany-browser
2) Place the Epiphany icon in the launcher and specify to "keep in launcher"
3) Close all Epiphany windows
4) Launch Epiphany from another application, say, via xdg-open
5) A new, ephemeral Epiphany icon is placed in the launcher, separate from the pinned one.
