[Bug 1379502] [NEW] Volume is not remembered when using optical output

Public bug reported:

After a reboot, if the selected audio output is not the default analog
output (ie optical or coaxial) the volume set by the user is forgotten
and so the volume is set to maximum.

This is a serious problem because it can damage hearing and equipment.

If nothing else the default should be minimum volume, not maximum.
Maximum volume should never be the default for anything.
