Hi All,

I am trying to develop this form in the DHIS2 but my greatest worry is
if the DHIS2 can have pictures inserted against each gCHV data.

Hi Stephen, pls see attached the tools being used to conduct the
community assessment. The excel sheet is to be used by the National
Coordinators while the CHV and community profile forms will be used by
the data collectors.
