Hi Stephen,the form is perfectly possible to implement in DHIS, except the picture. This is something that you would solve with an HR management system, like iHRIS.
This would also be more appropriate for the on-change updates, such as a CHW receiving training or a new bike, though this can also be done if you implement it in DHIS2 as a single-stage programme.
Is this a one-time event assessment, or is the form something to be kept up-to-date? If one-time event, I would do it in DHIS2 (without the picture), but if this is the first of many forms for HR management, I would consider iHRIS, and I know Theo and Bal Ram are thinking along these lines also. As said, you can keep this a "living form" in DHIS2 also, but I suspect it might be better to explore iHRIS as I know USAID are looking for an HR management system for MoHSW.
