Hi Yoann and Gavin,
A custom form or an app may achieve that for you. You need to use the
Api,SQL views,some jquery/angularJS. In this way, the location of the event
(clinic level) can be just an attribute/dataelement which can be written
back to the database through api.
HISPUG implemented something similar for Rwanda for handling outbreaks and
alerts.
