but how to add the first prog with register? when you put the program with register, you need a register program at first.

When you use the relationship option, you  need to have two programs one main and the other child. To choose program to relate to at "Program for new relative to be enrolled in *" you need to have another program with registration, seems you dont other programs with registration in your instance.
